Initial Enquiry: When you contact us about our services, we collect your name, email address, phone number, and any other information you choose to provide so we can respond to your enquiry.
Booking an Appointment: Before your first session, we will ask you to complete an Initial Consultation Form. This may include:
- Name, address, date of birth, and contact details
- Occupation and GP contact information
- Next of kin information
- Relevant medical history and prescribed medication
- Reasons for seeking our services and other relevant background information
We use the Initial Consultation Form during your first appointment to collect data about your general emotional well-being, the reasons for your visit and any other information you may want to share with us.
During Sessions: We may take notes on your emotional well-being and information you share to provide professional and safe therapeutic services.
Some of the information we collect may be sensitive personal data (special category data). By providing this data, you give explicit consent for us to process it as described in this policy. The legal basis for processing your data is to fulfil our obligations to you under our contract and to deliver our services safely and professionally.
2. Confidentiality and Disclosure
All information you share with us is confidential. We will not share your personal data with third parties unless:
- Required by law
- There is a risk to your health or safety, or the safety of others
In cases where there is a significant risk, we may disclose information to your GP even without formal consent.
We may also share anonymous case studies with supervisors or peer support groups for professional development purposes. This does not breach confidentiality and ensures we provide you with the best possible care.
3. Storage of Data
Your personal data, including session notes, is stored for up to 8 years in secure formats, either electronically or in hard copy.
We implement strict organisational and technical measures to protect your data from unauthorized access, loss, destruction, or damage. All personal data will be held for as long as is necessary in accordance with all applicable UK legal, financial and regulatory laws.
Important: Transmission of information via email or the internet is not completely secure. By sending data electronically, you do so at your own risk. Please contact us if you prefer a more secure method.
Your personal data will not be transferred outside the UK.
4. Accessing and Updating Your Information
Under UK-GDPR, you have the right to request access to a copy of the personal data that we hold about you via a Subject Access Request (SAR). We will comply with the information request, as UK-GDPR requires, within one month from the day that we receive the SAR. You have the right, subject to exemptions, to ask to:
- Correct or update information if it is inaccurate. Please contact us to ensure that personal data is regularly updated, especially contact information such as: email address, phone number and home address.
- Request that we stop processing your data, where not required to do so by law
- Object to the processing of your data
- Obtain your data for use across other service providers (data portability)
We will not share your personal data with third parties without your explicit consent unless legally required or necessary for safeguarding purposes.
